• New Client Appointment Request

    If you would like to request your initial appointment, please click on the "request appointment" link below.  Existing clients may continue to request appointments using their personal client portal or click on the "intake and assessment" to proceed.

  • At Serendipity we care about and respect your time. To make the intake process simpler, we have streamlined the process by incorporating the services of SimplePractice (c), an online practice management system. Prior to your initial session, you will be provided access to your personal client portal where you will complete your intake documentation. Documents to be completed online include the Consent Form, HIPPA Notice of Privacy Practices and Client Questionnaire.  All documentation provided will be kept and maintained in the strictest of confidence in compliance with all applicable laws.

  • Confidentiality

    The law protects the privacy of all communication between a patient and a therapist.  In most situations, I can only release information about your treatment to others if you sign a written authorization form that meets certain legal requirements imposed by HIPAA. There are some situations where I am permitted or required to disclose information without either your consent or authorization. If such a situation arises, I will limit my disclosure to what is necessary.  

  • Insurance

    We are currently In-Network providers for Cigna Behavioral Health and Cigna EAP.  

    You may however qualify for Out-of-Network reimbursement which can be determined by calling your insurance company and asking about Out-of-Network Mental/Behavioral Health Benefits. Detailed statements of service, known as Superbills, are provided with each session and can be submitted to your insurance provider for possible reimbursement.   

    Questions to ask your insurance company:

    Do I have mental/behavioral health insurance benefits?

    Do I have “out-of-network” mental/behavioral health benefits?

    What is my deductible and has it been met?

    How many sessions per year does my health insurance cover?

    What is the coverage amount per therapy session?

  • Cancellation Policy

    We fully understand and appreciate that unexpected events occur which may interfere with your ability to keep you scheduled appointment.  Nonetheless, we require that you provide 24 hour notice when cancelling or rescheduling an appointment to avoid being charged the full session fee for the missed appointment.
